Please include in your message at least
- your name and date of birth;
- (where applicable) the consult number or the date of the consult;
- a clear description of your complaint;
- what you would like to achieve with your complaint.
After receipt
- we confirm your complaint within five working days;
- you receive, within six weeks at most, a written response with reasons, setting out our judgement on your complaint, any measures we will take and the timeframe within which they will be carried out.
If more time is needed (for example because further investigation is required), we may extend this period once by no more than four weeks; we will inform you of this in writing.
